As a hospital based skilled nursing unit, our patients are treated by physicians who round daily and lead personalized treatment plans—ensuring hospital-level care at every stage.
Our integrated services, including on-site radiology, laboratory testing, and pharmacy, as well as other physician specialists as needed to support a seamless recovery experience.
Therapy at Cornerstone begins early, even while patients are receiving medical care. We offer physical, occupational, and speech therapy tailored to each individual—no rigid schedules or minimum requirements. Every care plan is built around the patient’s goals, energy, and abilities, because meaningful progress starts with understanding where you are and guiding you forward from there.
Our hospital-based skilled nursing unit (SNU) provides a higher level of medical and rehabilitative care than a typical nursing home, serving as a transitional step between a hospital stay and returning home. The unit is staffed by licensed medical professionals and focuses on short-term recovery from a serious illness, injury, or surgery.
Key differences of a hospital-based Skilled Nursing Unit
Compared to a SNF/Nursing Home, a hospital-based unit offers some distinct features:
· More complex cases: Hospital-based units often serve patients with more severe or acute conditions, such as stroke rehabilitation or post-operative recovery.
· Higher intensity of care: A hospital SNU provides more frequent access to doctors and specialists and may offer more intensive therapy programs than a freestanding facility.
· Convenience: Being located within a hospital allows access to physicians, respiratory therapists and Nurses who are adept at managing complications and that help prevent returns to a short-term acute care hospital.
