  • Patients who need intensive therapy

    To qualify for inpatient rehab, patients typically must be able to participate in: At least 3 hours of therapy per day, for 5–7 days per week. This may include physical therapy (PT), occupational therapy (OT), and speech therapy (SLP).

  • Patients who need low- to moderate-intensity rehabilitation

    SNFs provide therapy, but not at the intensity of inpatient rehab. Appropriate patients typically:

    • Can tolerate 1–2 hours of therapy per day, several days per week

    • Need support with mobility, strength, balance, or activities of daily living (ADLs)

    • Post-surgical recovery (e.g., hip fracture, joint replacement)

  • Yes. Cornerstone Post Acute Care provides 24/7 nursing and respiratory care, along with daily physician involvement and frequent specialist consultation based on patient needs.

    Medical equipment is provided by the hospital.

  • A physician referral is usually required. A clinical liaison or case manager from Cornerstone Post Acute Care reviews the patient’s medical condition to determine eligibility.

    Med Surgical floor - There are no set visiting hours; however, please note that our front doors are locked between 10:00 PM and 6:00 AM. If you plan to stay overnight with your loved one, please ensure you are inside the building before 10:00 PM, as re-entry will not be available until morning.

  • Yes. We accept most insurance plans, and our team verifies your benefits during the admissions process. We encourage families to confirm coverage directly with their insurance provider, but our staff is available to help answer questions and provide referrals if needed.

  • Yes. If your doctor has attending privileges at our hospital, they are welcome to continue managing your care. If not, we will match you with one of our experienced physicians. We also communicate updates to your doctor to ensure your care remains continuous and well-coordinated.
