Our Story

At Cornerstone Healthcare, we were founded on a simple but transformative idea: that the road to recovery shouldn’t be fragmented or confusing. That patients deserve care that follows them, not care that requires them to start over with every new diagnosis or doorway. Our integrated campus model brings together Long Term Acute Care (LTACH), Inpatient Rehabilitation (IRF), and Skilled Nursing (SNF)—a full continuum of post-acute services, housed under one roof.

This isn’t just convenient—it’s compassionate. It means that families can worry a little less. It means providers can trust that the handoff will be more of a handshake. And it means that patients move forward, not sideways.

With every level of post-acute care available in a single location, we offer patients, families, and referring providers something rare in today’s healthcare landscape: continuity, clinical consistency, and peace of mind.

Our Mission

To deliver coordinated, high-quality post-acute care that evolves alongside our patients—without ever interrupting the rhythm of their recovery.

Our mission is rooted in trust, safety, and dependability. We train our staff to lead with both clinical excellence and heart, because we know that the best outcomes begin with a human connection. And while no two patients are alike, every one of them deserves the same thing: to feel seen, supported, and empowered to reclaim the life they love.

This is the Cornerstone difference. Exceptional, connected care that meets each patient exactly where they are—and moves with them as their needs evolve.
